Report An Incident

Should an incident occur on your premises it is imperative that you administer first-aid and address this situation immediately. It is also imperative that you keep a record of all incidents including your responses and names of any witnesses. Please use one of the forms below, and fax or email to our office at 601-853-6141 or claims@sportsfitness.com.
